The self-hatred and self-castigation of anorexia can aggravate depression, and depression can in turn aggravate self-hatred. Isolation can also aggravate depression. A person who is already besieged by self-defeating thoughts is apt to ruminate when uninterrupted by the natural distractions provided by other people. Anorexics tend to withdraw and to self-isolate, in part to avoid the painful selfjudgments and self-defeating comparisons that they are prone to generate in the presence of others. However, what feels self-preserving may actually aggravate pessimistic mood.

If an anorexic’s compulsive behaviors extend to areas beyond food and weight, and meet other criteria for frequency and persistence, she can be diagnosed as having obsessive-compulsive disorder. This is not unusual when an anorexic becomes progressively more anxious and upset with prolonged malnutrition. In these circumstances, more and more of her life begins to be preoccupied with thoughts about others, herself, her performance, how others see her, or what she is doing wrong. She develops an ever-greater repertoire of compulsive behaviors to quell the intrusive thoughts.

The main traits that characterize addiction are present in anorexia. These include continuation of behavior despite adverse consequences; lack of control; and the progressive nature of the disease. At first, an addict can generally manage the substance, behavior or drug. However, in time, the addict no longer maintains control: physiological reactions, anxiety, obsessions, distorted thinking, and impulsivity reign. So it is with anorexia. An anorexic initially chooses to restrict food intake, but she reaches a point where the choice is no longer hers; her disease takes over, and fear and anxiety drive her behavior.
